It is possible your belt is stretched and has developed a permanent twist due to broken cords.  If your idler and drive pulleys are lined up OK, lay the belt out and look for a twist or bend in the spine of the belt.

  My father has a cub with a woods 42" mower and we can not keep the belt on it. We have replaced the belt and tightened it up as tight as we can get it but the belt still jump off. We have checked all the bearings and they are all good. We are stumped is there anyone else who has run across this problem and any suggestions would be helpful Thanks.
