Appears to be a 6207 shielded, both sides I would assume. No zerk. McMaster.com if someone is away from a store. I’d get sealed both sides but shielded is OK as well. Very common size in 15HP electric motors on the fan end. We had a drawer full of bearings at the wine factory.

Spencer, I don't want to brag, but I think I have one or 2 of those in my bearing stash.   Back in the 90's when Bailey's had a lot of cheap bearings I ordered about $2000 worth of bearings to have in case something broke.  Now the problem is just finding them....

This model was manufactured from 1968-1972.  Judging by the paint it is original equipment, so I guess 47 to 51 years of service is acceptable :-)
