According to the news article it must have been an accident regardless 
of how careful he was. His foot slipped off the clutch pedal while it 
was still in forward gear. Likely the tractor reared up before he could 
hit the clutch again.
Better to keep it at low throttle for this type work so that the engine 
might stall before it flipped over. I don't know if that would have 
saved him but its my first thought.
