Ralph,  if he had hooked the chain on the actual drawbar, the one down under 
the tractor not the bar that fastens to
the lower 3pt arms and not the top link, it wouldn't have happened period. 
As long as the attachment point is
below the axle the tractor will loose traction before it can come up that 
high.  That  is assuming he wasn't pulling
up a steep slope.

According to the news article it must have been an accident regardless
of how careful he was. His foot slipped off the clutch pedal while it
was still in forward gear. Likely the tractor reared up before he could
hit the clutch again.
Better to keep it at low throttle for this type work so that the engine
might stall before it flipped over. I don't know if that would have
saved him but its my first thought.
