you might want to read the NASA report on "Tin Whiskers". Turns out an old problem with the formation of tin whiskers was likely the Toyota problem. I posted the link in a previous post. The formation of the tin whiskers was documented by NASA and was not identified during tests by others due to destructive testing. The black box would show 100% accelerator but that was due to the accelerator sending unit tin whisker issue.

Fair enough, I remember back a couple years ago Toyota had a similar problem,
NHTSA sampled some of the cars in question and analyzed the "black box",
accelerator was at 100% brakes 0%, driver error. Mike

It wouldn't do it, Mike. All the normal controls went bonkers! Wouldn't shift
into neutral, wouldn't turn off, wouldn't slow down. None of the regular
mechanical connections that we all think of as a part of a car existed on the
model she was driving. It would be like running a Diesel with the governor
disconnected.
