If it is a 7.3 Powerstroke, the most likely causes would be either a head 
gasket or an injector tube leaking. These heads rarely crack.

If it is an older 7.3 or 6.9, probably a head gasket or cracked head.  A pin 
hole in the cylinder is more likely to get coolant in the oil rather than out 
the exhaust.
