[Steam-engine] Small hand hole plates

If you are cutting openings for hand-hole you will
need to find a National Board approved "R" shop to do
alteration calculations.  The would be required
whether you put hand-holes in, or put in a larger
half/coupling  for clean out holes.  Just tapping
bigger holes in the plates may not have enough threads
to be in compliance with ASME boiler codes.
